You will need to tick next to each section to indicate that you fully understand it. Once this form has been emailed to you, it must be completed and sent to us at least 1 week before the treatment date.

• I understand that I am a model for a training course where students will be performing the procedure on me. When modelling for a beginners course, I acknowledge that the students will have carried out their theory element of the course before performing the treatment on me. When modelling for an advanced course, I acknowledge that the students are qualified PMU artists who are learning a new treatment or technique. They will be closely supervised by Faye, an experienced permanent makeup artist.
• I understand that I am a case study model who will have a procedure performed by a student that has completed the course, but needs case studies to complete their qualification.
• I agree that it is my responsibility to complete my patch test (with the exception of tattoo removal and facial courses) and my medical questionnaire, and contact Faye if there is anything I have ticked on the questionnaire that may interfere with having the procedure.
• I understand that if I fail to mention any medication on the medical form and it means that the treatment cannot go ahead on the day, I will forfeit my treatment cost.
• I understand that it is my responsibility to plan my journey so that I arrive at the clinic 10 minutes before my appointment and allow at least 4 hours for the permanent cosmetic procedure to be carried out. For facials or tattoo removal procedures, I will allow for 2 hours.
• I understand that I need to arrive without makeup.
• I understand that this is for 1 treatment and if I require a follow up appointment, it will be with a qualified technician that works for FM Permanent Cosmetics Ltd, at a cost of £65. The follow up appointment must be carried out within 3 months of the original treatment and anything outside of this time will be charged at the full rate charged by the particular technician.
• I understand that I must pay the correct amount for my treatment. For permanent cosmetics, it is £100 for a student model or £150 for a demo model. For tattoo removal, I understand that it is £25 for a student model or £50 for a demo model. This is due upon booking my treatment date and is not refundable or transferable.
• I consent to the taking of photographs/videos before, during, and after the procedure. These may be used for educational purposes, social media, training promotion or documentation of the training process. I understand that my images or video footage taken can and may be used by the students taught by FM Permanent Cosmetics Ltd.

By agreeing to participate as a model/case study for a student training course, you confirm that you have read, understood, and agree to abide by these terms and conditions. You also acknowledge that any breach of these terms may result in termination of the treatment without compensation or liability on the part of FM Permanent Cosmetics Ltd.
