• Prices are correct at the time of going to press. Prices are subject to change without written notice.
• The course model booking is confirmed upon receipt of the completed registration form and full payment of the correct model fee (demo or student). Payment must be made in full before the course start date (via bacs transfer) and is non-refundable.
• Please note that cancelled appointments will only be recognised with written confirmation (email, text or special delivery post). Booking payments received by Faye is verification that full information has been provided regarding our terms and conditions, in the event of any cancellation.
• Additional top up sessions maybe required depending on many factors including but not exclusive to, skin type and condition, lifestyle, environmental factors and medication. Due to the nature of the treatment, 100% success for colour retention cannot be guaranteed. All top up treatment appointments must be taken 4 to 6 weeks after the original model procedure, with a minimal fee of £65. Any appointment made after this time is chargeable at the rate stated on the booking system. If you do not return for all of the treatment sessions or follow the care advice (preparation and aftercare) as set out in the treatment plan, you accept total responsibility for the final result.
• We recommend an annual colourboost to maintain a ‘fresh look’, which is offered at a lower price within 18 months of your first treatment. Colourboost procedures are for 1 appointment only unless otherwise stated.
• By receiving a treatment as a training course model, you acknowledge that the procedure will be performed by a student under the supervision of Faye (a qualified instructor). You understand that the student is in training and accept the potential risks associated with receiving treatment from someone who is still learning.
• You acknowledge and accept that there are inherent risks involved in any cosmetic procedure, including infection, allergic reactions, and dissatisfaction with the results. Faye and her students are not liable for any adverse effects or complications that may arise from the treatment.
• FM Permanent Cosmetics Ltd cannot perform any enhancements on minors or have minors in the treatment room during an appointment.
• Please note that some treatments require a patch test to be done 48 hours prior to your appointment.
• Please contact FM Permanent Cosmetics Ltd, if you have any medical conditions or are taking any prescription drugs. Some medical conditions or drugs/vitamin supplements may have contraindications. A doctor’s consent form must be completed prior to your appointment if this is the case.
