These forms will be sent to you via email when you book your appointment and are to be filled out electronically at least 24 hours before your appointment date. As per our terms and conditions, failure to follow all written instructions or failure to complete the necessary medical forms prior to your allocated appointment, may prevent Faye or Laura from carrying out the treatment on the day. This will result in postponing the treatment and you will be charged 75% of the full procedure price.
